Full registration

To register with the practice all you need to do is complete a registration form obtainable from the practice reception.

Please note: you will be able to see a member of the practice team once the registration form has been handed to the practice, however, because it may take several weeks before your medical records are forwarded to us from your old doctor, newly registered patients are required to make an appointment with the practice nurse for a  health check to provide us with essential health information. The registration process is not complete until the application has been processed by ABMU Health Board Trust.
